The Federal Reserve has issued rules that restrict charging overdraft fees on debit card and ATM transactions, without your express consent. This includes debit card purchases you make at a store, online, or by telephone. This form is designed for you to provide consent for this service.

We encourage you to read the information below carefully, so that you will make an informed decision regarding this service.

• By submitting this request, you are authorizing Safe 1 Credit Union to overdraw your account if there are insufficient available funds to pay and process ATM withdrawal requests and everyday debit card transactions and charge a fee for each overdraft.
• If we decide to pay an overdraft, you will be charged the following fees:
  We will charge you a fee of $30.00 each time we pay an overdraft
  There is no limit on the total fees we can charge you for overdrawing your account
• We offer other ways of covering your overdrafts that may be less expensive, such as an Overdraft line of credit loan. If you do not currently have an Overdraft line of credit loan, contact us to learn more about this option.
• If you are currently enrolled in our Courtesy Pay program, we may still pay your overdrafts for other type of transactions, including checks, whether or not you request overdraft coverage for ATM withdrawals and debit card transactions.
• You may revoke this coverage at any time. You may revoke coverage in person or by calling us at (661) 327-3818. Enrollment does not guarantee we will pay your overdrafts and we may revoke this service at any time.
